Real-life Case Story- an eye opener
Real-life Case Story:
π
“A 50-Year-Old Walked in With Fatigue. Here's What We Found.”
π
Fatigue isn’t always from a busy day. A simple blood test revealed a silent duo
working against his energy: anaemia and hypothyroidism. Here's the real-life
case.
A Tired
Tale — But Not from Work
A 50-year-old gentleman, let’s call him Mr.
Raj (name changed), walked into my clinic complaining of chronic fatigue. He was a working
professional, managing a moderately active lifestyle. But for the last 6
months, he had been feeling “just off.”
No major pains. No weight loss. Just an overwhelming tiredness.
“I sleep, but I wake up more tired,” he said.
Subtle
Clues We Almost Missed:
Upon further discussion, Mr. Raj
mentioned:
- Craving tea 3-4 times a day to “stay active”
- Breathlessness while climbing stairs
- Occasional constipation
- Feeling cold even when others didn’t
On examination:
- Pale lower eyelids
- Slight swelling around the neck
- Brittle nails
The Lab
Report Tells the Story
π©Έ Blood tests were ordered — and here’s what we found:
- Haemoglobin: 9.2 gm/dL (Low)
- TSH: 8.5 mIU/L (High)
- Serum Ferritin: 15 ng/mL (Low)
✅ Diagnosis:
Iron Deficiency Anaemia with Subclinical Hypothyroidism
Upcoming- “Subclinical
hypothyroidism — a topic I’ll
elaborate on in an upcoming blog very soon."
Connecting
the Dots:
Both anaemia and hypothyroidism can cause
fatigue — but together, they amplify the exhaustion.
Low iron also affects thyroid function. And in return, low thyroid slows gut
absorption, creating a vicious cycle.
What We Did:
Mr. Raj was started on:
- Low-dose thyroxine replacement
- Oral iron supplements with vitamin C
- Diet changes (more green leafy veg, citrus fruits, jaggery)
Follow-up in 6 weeks showed remarkable energy
improvement. Haemoglobin rose to 11.5 and TSH started normalizing.
Takeaway for Readers:
π Fatigue is a signal, not a lifestyle consequence.
π Get your blood work done if fatigue lingers beyond 3-4 weeks.
π§ Think beyond stress — your body might be telling you something deeper.
π¨⚕️ My
Note:
If you're often tired, don’t ignore it.
Fatigue might be your body whispering before it starts screaming.
